The answer depends on your treatment plan, but the most important thing is that you’re always in control of what happens in your session.
What to Expect During a Pelvic Floor Therapy Session
During your first visit, your pelvic floor physical therapist will likely…
- Ask about your symptoms, history, & goals
- Examine your posture, breathing, & core
- Perform a physical assessment (external and/or internal)
- Teach you strategies and exercises tailored to your needs
The therapist will explain each step clearly, so nothing catches you off guard.
When Do You Need to Undress?
You usually don’t need to fully undress unless you choose to participate in an internal pelvic floor muscle exam.
External-Only Assessment
You stay fully clothed or may wear loose shorts for movement-based evaluation. Your therapist might look at your hips, abdomen, or posture while you’re fully dressed.
Internal Assessment (Optional)
If you and your therapist decide an internal exam would help, you’ll remove your pants and underwear in private. You’ll lie on a treatment table and use a drape or sheet to cover your lower body. Your therapist will only begin the exam after you give clear, verbal consent.
Note – You decide whether or not to do an internal exam. You can say no at any time before or during the session.
Do You Have to Do an Internal Exam?
No. Internal exams are helpful for understanding muscle tension, strength, or coordination, but they’re not required. Many people begin therapy with external treatment and get excellent results.
What If You’re on Your Period?
Being on your period doesn’t automatically cancel your session. You can still receive care and even choose to proceed with an internal exam if you’re comfortable. If not, you can easily reschedule or stick with external therapy.
