How Painful Is Pelvic Floor Therapy?

How Painful Is Pelvic Floor Therapy?

It shouldn’t be. While some moments may be uncomfortable, especially in the beginning, your therapist is trained to work within your limits & make each session as gentle & empowering as possible.

What Does Pelvic Floor Therapy Involve?

Pelvic floor therapy typically includes…

  • A thorough evaluation (posture, movement, breathing, and muscle strength)
  • Gentle manual therapy (external or internal, depending on your needs)
  • Targeted exercises
  • Breathing & relaxation techniques
  • Education on bladder/bowel habits, posture, & home exercises

Each part of the session is personalized to your body & your comfort level.

Is the Internal Exam Painful?

Some sessions may include an internal pelvic floor muscle assessment, performed vaginally (or rectally in rare cases). It’s always done with your consent & explained beforehand.

Most patients describe the internal assessment as slightly uncomfortable, but not painful. The therapist uses one gloved, lubricated finger & works gently to assess muscle tone, coordination, & tenderness.

If you experience pain, your therapist will stop or adjust immediately. This is a collaborative process, not a forced one.

When Might You Feel Discomfort?

Mild soreness or tenderness can happen during or after…

  • Manual release of tight pelvic muscles
  • Stretching areas that haven’t been moved in a while
  • Beginning a new set of exercises
  • Trigger point therapy for chronic pelvic pain

This is similar to the sensation you might feel after a deep tissue massage or workout, temporary & usually followed by relief.

How We Minimize Discomfort at Physical Therapy by Phoenix

At Physical Therapy by Phoenix, your comfort & trust come first. Here’s how we make your experience positive…

  • Trauma-informed care – We work with many women who have experienced trauma, surgery, or chronic pain. You’re in control every step of the way.
  • Clear communication – Nothing is ever done without your permission. You can pause, skip, or stop anything at any time.
  • Individualized pace – We start slow & work up only as your body is ready.
  • Soothing environment – Our clinic space is calm, private, & welcoming.

Who Typically Feels the Most Sensitivity?

You may experience more sensitivity if…

  • You have chronic pelvic pain or vaginismus
  • You’re early postpartum & tissues are still healing
  • You’ve had trauma or surgery in the pelvic area
  • Your muscles are especially tight or guarded

In these cases, therapy is even more gradual & supportive, focusing on nervous system regulation & pain-free progress.

When Should You Be Concerned?

While some discomfort is normal in the healing process, sharp pain, worsening symptoms, or emotional distress are not. Let your therapist know immediately so your plan can be adjusted.
